Responsible for case management to assist clients with housing, employment, finance, education, legal and family issues. Responsible for assisting in the development and maintenance of community rituals and events that encourage recovery in an alcohol and drug free living environment. Provide resources, support, and counseling to program participants to facilitate successful family recovery goals. Must be respectful, culturally competent and be able to work in a confidential environment.
Bachelor’s degree with an emphasis in social work or certified SUD Counselor. Experience in the alcohol and drug addictions field and or in outpatient or residential treatment program; State certification for substance use disorder counselor.

The following responsibilities represent the essential functions of the position. An employee in this classification is responsible for carrying out the following functions:
Acts as a resource for alcohol and drug addiction information, treatment, education, and/or prevention, program development, community activities, acts as a liaison between program services and the community.
Coordinate and provide referrals and case management for housing, medical, financial, vocational, legal, educational, and peer support necessary for long term recovery.
Teach classes and/or provides one-to-one education. Classes may include life skills, group peer-to-peer, relapse prevention, parenting workshops, outpatient services, or other.
Conduct or coordinate crisis intervention to address substance abuse of residents.
Provides oversight of daily operations and services including case management, communications, and advocacy, assures quality services are provided in timely manner.
Monitor and document Family Recovery Plan goals; Case Management documentation in case files; including but not limited to program stipulations that jeopardize their housing agreements, service delivery, resident participation in meetings, community events and recovery programs.
May conduct client intakes. Monthly staff meetings; Act as advisor on Community Council Committees.
Assures compliance with all safety regulations, and agency policies and procedures.
Assures compliance with all client confidentiality requirements.
Assist clients to secure long-term housing opportunities; apply for rentals, housing vouchers; assist clients to improve their financial status.
Procure employment opportunities for men and women in recovery within the local community.
Assess readiness through volunteer activities. Provides client assessments for job match including, transportation possibilities, the client’s work interest, and experience and other relevant factors. Provides support in finding external resources as needed.
Develop and hold weekly “Job Club” with clients looking for work and clients recently placed. Assist clients in preparing resumes, applying for positions on-line and in person.
Responsible for maintaining a professional relationship and assure timely communication with employers, landlords, clients, staff and funders. Represents Sun Street Centers in the community, including presenting to local service organizations. Works closely with the Housing Authority, Social Services and Behavioral Health.
Required knowledge, Skills and Abilities:
Proficiency in MS Word, Excel, and databases
Ability to communicate effectively and provide excellent customer service with children and adults.
Ability to successfully perform individual and group counseling.
Ability to observe, evaluate, document, and communicate verbally and in writing.
Knowledge of alcohol and drug addiction treatment, recovery, 12-step recovery process, and the effect of alcohol and drugs within family systems.
Knowledge of community resources.
Ability to communicate and lead in a culturally competent and sensitive manner.
Constant movement around service area, and travel in the community. Time spent sitting, using a computer station, periods of intense concentration, using the telephone, paperwork. Time spent in meetings. Exposure to some outdoor weather and environmental conditions.
NOTE: Employees will be required to be fingerprinted and will be subject to clearance by the Department of Justice.
