Assistance providing appropriate meal planning and nutrition as directed by Client’s physician, nurse or other person qualified to determine nutritional needs and recommendations
Assistance with laundry and light housekeeping
Assistance with grocery shopping and meal preparation
Assistance with re-ordering medications as prescribed by Client’s physician or physicians
Assistance with reminding client to take prescribed medication, with the understanding that PHC may only remind Client to take medications, but the Client must be the only one to physically handle and take the medications
Assuring safety at all times, fall prevention, injury prevention;
Support for individuals needing assistance throughout the night, including help getting to bed, toileting, personal hygiene, caregivers are available to assist with after bedtime transfers and redirection when someone may get confused

When Staying in a Hospital or a Skilled Nursing Facility
When your loved one needs additional support in a skilled setting, we can help. We are able to work in any facility and have partnerships with skilled facilities around the county. This support can be fall prevention, redirection for someone who is confused, assisting with feeding and just more one on one care.
We can provide additional entertainment with you’re loved one, play cards, read books, take to activities and help them to engage more.
